Warning - this doctor is not transparent about costs; families should ask for complete pricing!! She will tell you what is wrong then not give you ANY indication of how to get better unless you subscribe to her monthly program ($$$$)! My son has been struggling with serious health issues for years, so we were very hopeful when we started with Dr. J. We spent about $2,000 on testing, believing that treatment would follow once results came back. Instead, when the results arrived, we were then told we needed to pay another $2,000 and commit to a care program where his treatment would begin. None of this was explained during our initial visits. We were never told we wouldn’t receive treatment after paying for testing unless we paid more. After 2 months of waiting for test results and her availability, it was heartbreaking to tell my son that we still had no solutions (ZERO) and had to find a new provider. He was crushed as Dr J had worked to connect to him - we trusted her. If you have budget constraints, I strongly recommend asking for the full financial picture ahead of time (testing + treatment + timeline). The lack of transparency made my sons issue even more of an uphill climb for us. When I called to see if there was anything they could do even just provide us with a supplement list and a specific diet direction to go to help while we’re in a financial need they would give us nothing. I asked them to send me the email or any communication where they showed that we would not actually get treatment after testing and to this date I have received nothing. Be very careful with this doctor.

Dr. Jaye is brilliant and very knowledgable in helping to heal problems from the inside out. Valerie manages the office and is very responsive to phone calls, messages, and helping with questions. After just a few months of care and treatment I am feeling so much better.

I have had GI issues for 18 years. I lost my gallbladder, and almost had to have part of my colon removed a few years ago. After a summer of hospital stays, several CT scans, MRIs, colonoscopies, endoscopies, biopsies, and blood work, I started a journey to heal by hiring a holistic nutritionist. Although my efforts were good, I still wound up on steroids 3 times last year because of inflammation of the ilium. I was at my wits end, fighting my GI doctor who wanted to put me on Crohn’s infusions even though, after all of his expensive tests, he couldn’t even say that I definitely had Crohn’s. In May, after yet another colonoscopy, endoscopy and biopsies, he wanted to run another MRI. I decided I was done with all of the same tests that never gave me answers. I was determined not to ever take the infusions. That’s when I found Dr. Jaye at Forum Functional Health. She spent so much time with me asking all of the important questions to learn about my issues. On top of the digestive issues, I had weight gain, peri-menopause and all of the symptoms that go along with that amazing part of life like fatigue, low energy, hot flashes, brain fog etc. She had me take 3 tests at home and mail them to the labs. The results were not surprising. There were so many things contributing to my digestive and hormonal issues, which in turn made it impossible to lose weight and even contributed to major weight gain over the years. What really surprised me was that I had 3 bad bacteria that were not detected in all of the big expensive tests I had done in the past. I started on the protocol that Dr. Jaye prescribed for me and within a month I started noticing a change in my energy level. I started doing projects around the house that I just didn’t have the energy or motivation to do before. My family noticed a big difference in me and as a result, my husband, 2 daughters, my sister and my best friend have all started seeing Dr. Jaye too. It took even less time for my husband to start feeling more energy. I haven’t seen him do so many projects in the house in years! My youngest is going to see her in a few weeks for cystic acne and food allergies. I can’t wait to see how she can help him too. I can’t stress enough how happy I am that we have found Dr. Jaye. She listens, she cares, and she educates. I recommend her to everyone I meet. I can’t wait to see how great I will feel at the end of my 18 month plan. You really don’t know how bad you feel until you start to feel good again. Growing older doesn’t mean feeling bad. If you are feeling like I did, call her today! You deserve to feel good again!

I have been seeing Dr Jaye for about 1 year now and have received so much support from her through different life changes. I have seen changes in my anxiety and acne. I recommend functional medicine to anyone seeking support for any health or mental struggles you are going through!

My goal for 2024 was to get healthy from the inside out. I have PCOS and endometriosis and have been fatigued and bloated since I was a child. I started testing March 2024 and started the program in May once all my test results were back. I’m 8 mos into the program and have never felt better. My bloating is gone, no more intense cravings, and I finally have energy. In my first appointment, I ranked how I felt as a 7/10 which Dr. Jaye didn’t think was all that bad. But now reflecting back I think I was probably actually a 4-5/10. I had felt bad for so long that I didn’t realize JUST how bad I felt until I finally felt good. I can’t wait to see what else is going to change with my body as I continue to work the program!
